If you’re playing a team sport, you need to have a sports physical. Get a same-day sports physical and be ready to play.
$50 – No insurance billing
Your provider will go through the requirements on your sports physical form and will check for the following:

QuickCare has a location right across the street from the OSU campus and we’re set up to fit a student’s busy schedule.
We offer treatment for common illnesses, injuries, and more with a clear price list. QuickCare bills as a primary care service rather than urgent care. If you’re uninsured or prefer to pay out of pocket, we welcome cash, checks, and credit cards.
QuickCare offers instant online translation for many languages.
QuickCare accepts many insurance plans including, Providence Health Plans, Regence, Moda, Aetna, InterCommunity Health Network, Oregon Health Plan, and United Healthcare.

QuickCare bills as a primary care service. This offers a lower cost to you than if we billed as an urgent care.
Primary care provider is outside The Corvallis Clinic?
To keep the cost to our patients lower, QuickCare bills as a primary care service rather than as an urgent care. However, if your primary care provider is not affiliated with The Corvallis Clinic, your insurance might bill your visit as out-of-network primary care, which is a higher rate. If so, you might want to consider paying out of pocket to keep your costs lower. We welcome cash, checks, and credit cards.

Meet Our Providers
Lee Chamberlain, FNP-C
Ms. Chamberlain comes to QuickCare from Samaritan Health Services where she worked as Family Nurse Practitioner at Geary Street Family Medicine in Albany from 2015 to 2017 and at Samaritan Lebanon Health Center in 2017.
She is board certified by the American Association of Nurse Practitioners and earned a Master of Science in Nursing, Family Nurse Practitioner, Gonzaga University, in 2015.
Prior to becoming a nurse practitioner, Ms. Chamberlain was a registered nurse at Good Samaritan Regional Medical Center in Corvallis from 2006 to 2015; Iverson Memorial Hospital in Laramie, WY, from 2002 to 2006; and St. Mary’s Hospital in Tucson, AZ, from 1999 to 2002. She earned a Bachelor of Science in Nursing from the University of Arizona in 1999. Before deciding to become a nurse she was a mathematics teacher.
Ms. Chamberlain loves spending time with family and friends and enjoys traveling, hiking, knitting, and swimming.
Jennifer Kingsley, FNP-BC
Ms. Kingsley has worked as an independent contractor; for Harrison HealthPartners, in Washington state, in urgent care and family medicine; for the Jefferson County District Attorney, in Madras, Ore., as the designate medical provider; and for Mosaic Medical in Central Oregon.
She is a board-certified family nurse practitioner by the American Nurses Credentialing Center.
As registered nurse, she has worked at Good Samaritan Regional Medical Center, in The Corvallis Clinic Oncology and Hematology Department, and at Professional Home Care in Eugene.
Ms. Kingsley earned a Masters of Science in Nursing, Family Nurse Practitioner, from Gonzaga University, in Spokane, Wash., in 2012, and a Bachelor of Science in Nursing from Oregon Health and Science University, in 2008. She is currently pursuing a Doctor of Nursing Practice, Psychiatric Mental Health Nurse Practitioner, at Gonzaga.
Stephanie Nixon, FNP-C
Stephanie Nixon worked as nurse practitioner at Foothills Medical Clinic in Colorado Springs, CO, from 2014 to 2015. She was a registered nurse for nine years in Colorado and held positions as a charge nurse, a home-infusion nurse and nursing supervisor.
Ms. Nixon earned a Master of Science in Nursing from Walden University in Minneapolis, MN, in 2014, a Bachelor of Science in Nursing from Jacksonville University in Jacksonville, FL in 2011, and an Associate of Science in Nursing from Trinidad State Junior College in Alamosa, CO, in 2005. Ms. Nixon is certified by the American Academy of Nurse Practitioners.
She and her two children love to ride bikes, hike, ski, swim, and play sports.
Diana Scott, FNP-BC
Diana Scott, FNP-BC, received a Master of Science in Nursing, Family Nurse Practitioner from Oregon Health & Science University in 2008.
Ms. Scott worked as a Nurse Practitioner at The Corvallis Clinic from 2015-2017 treating patients for non-surgical conditions in Obstetrics & Gynecology and Urology.
She received a Bachelor of Science in Nursing from OHSU in 2006.
Leslie Shortridge, FNP-C
Leslie Shortridge, FNP-C, received a Master of Science in Nursing/Family Nurse Practitioner from Gonzaga University in Spokane, WA, in 2016.
Ms. Shortridge worked as a Nurse Manager from 2009-2014 at Pregnancy Alternatives Center in Lebanon, OR. She was also the Health Services Coordinator for Lebanon Community Schools from 2002 to 2008.
She received a Bachelor of Science in Nursing from OHSU in 1980.
What types of services do you provide?
QuickCare provides treatment of minor illnesses and injuries, preventive care, and sports physicals. It also offers immunizations and some laboratory tests such as strep, glucose levels, and urinalysis.
What if I'm late to my appointment?
If you are more than five minutes late, we will work to reschedule you later in the day.
What ID do I need to bring to my appointment?
If you are new to QuickCare and The Corvallis Clinic, please bring a government-issued photo ID, such as a passport, driver’s license, non-driver ID card or Department of Defense Identity Card. Also, bring your insurance card and any other medical information you feel might help you complete the demographic and medical history forms
If you are new to QuickCare but have been seen at The Corvallis Clinic, please bring a government-issued photo ID, such as a passport, driver’s license, non-driver ID card or Department of Defense Identity Card. In addition, please bring an insurance card if there have been changes to your coverage.
What if my insurance is not on the list?
We will accept cash or credit card payment. You may also use our receipt to submit a claim to your insurance provider.
Do you provide imaging services?
QuickCare does not offer x-ray or other imaging services. If needed, you will be referred to The Corvallis Clinic Imaging Services.

How early do I need to arrive for my appointment?
You should come five minutes before your appointment. If you are new to QuickCare and The Corvallis Clinic, you should come 10 minutes before your appointment to complete demographic and medical history forms.
If I need language assistance, can I be seen?
QuickCare offers instant online translation for many languages, including American Sign, Spanish, Arabic, Korean and Mandarin Chinese. Please call 541-754-1369 to schedule your appointment.
Can I schedule an appointment for family members?
Yes. When scheduling the appointment, please fill out the patient’s name.
Can I register people I'm not related to?
Yes. However, the email confirming the appointment will go to your email, not the email of the person for whom you are making the appointment.
Do you see children?
We see children ages 2 and older. Our providers believe pediatricians offer children under 2 years the best level of care. To make an appointment with one of our pediatricians at The Corvallis Clinic, call 541-754-1278. Pediatrics is open Monday through Friday from 8 a.m. to 6 p.m. and on weekends from 10 a.m. to 2 p.m. Same-day appointments are available. New patients are welcome.
How do I cancel my Appointment?
If you have made an appointment and used an active email address you should have received an email with a cancellation link. Click the link, and select cancel and your appointment will be removed.
